Facilitate Intake CHADIS offers a comprehensive choice of pre-visit online data-gathering tools including parent and teen goals, visit priorities, past medical, pregnancy-perinatal, developmental, social and family history.

Facilitate Triage and Diagnostic Documentation CHADIS offers screening for a wide variety of co-morbid disorders and developmental difficulties to triage and diagnostic tools to document DSM-5 criteria and severity such as CANS.

Teen Self-administered Tools Teens complete tools under secure login and results are not visible to parents or others.

Facilitate Symptom Monitoring CHADIS includes a range of tools to monitor symptoms between and before visits of: ADHD; depression; anxiety; tics and movements; and medication side effects.

MemoryBook Care Portal A secure individual website is created for each patient pre-populated by milestones from screening questionnaires to which photos or comments can be added and shared by invitation. Patient-specific text and videos for education or intervention, apps and support groups, and national and your local resource listings are displayed and searchable.

Add Revenue CHADIS evidence-based tools are usually paid by insurers under codes 96160, 96110, 96127; and some under 96111 assessments e.g CARS, CASD.

Communicate with Schools and Primary Care Providers CHADIS allows parents to provide online consent and “invite” teachers or other professionals to provide data such as standard ratings. ADHD ratings from parents and teachers are displayed in a graphical format to share with families. Results can be shared securely with primary care offices.
