Questionnaires for ADHD care for parents
• Parent Vanderbilt Assessment Scales open | close
There are now 4 versions of the Vanderbilt available in CHADIS: the initial and follow-up versions for both parent report, as well as alternative versions of each of these questionnaires that include the medication side effects items described below.
• CHADIS-DSM open | close
This is a diagnostic questionnaire we recommend for use in children being evaluated for ADHD because it results in presumptive DSM-4 disorder and DSM-PC  problem, or variation diagnoses (from AAP manual for primary care mental health classification) and a prioritized list of parent concerns.  Parents review a list of mental health symptoms and indicate their concerns and then prioritize these concerns.  Parents are then led through algorithms based on the logic of DSM criteria and are dismissed from the process as they meet or fail to meet diagnostic criteria.  CHADIS-DSM provides documentation of specific criteria that can justify diagnoses and covers ADHD and all comorbid diagnoses as recommended.  Briefer questionnaires available in CHADIS (e.g. Pediatric Symptom Checklist) may be used as an alternative but do not satisfy AAP recommendations to use DSM criteria and rule out comorbid diagnoses.
• Medication Questionnaire open | close
This tool collects information on medication prescribed, dosages and compliance.
• Medication Side Effects Questionnaires (Parent Version) open | close
Adapted from the side effects section of the Vanderbilt scales, these questionnaires assess potential medication side effects
• Child and Family Cardiac History open | close
This questionnaire assesses the history of cardiac problems in both the child and their family in order to implement new national guidelines for identifying those with higher risk of cardiac side effects.  In particular, this tool determines who should be considered for an EKG or cardiology consultation prior to the initiation of stimulant medication.

Questionnaires for ADHD care for teachers
• School Interventions Questionnaire open | close
This tool assesses special school services received (IEP, preferential seating, etc), plus a history of school disciplinary actions (suspensions, detentions, etc).
• Teacher Vanderbilt Assessment Scales open | close
There are now 4 versions of the teacher Vanderbilt available in CHADIS: the initial and follow-up versions and alternative versions of each of these questionnaires that include medication side effects items. Medication side effect items can also be assessed by the separate questionnaire shown below.

A separate questionnaire with the same side effects questions as in the comprehensive Vanderbilt scales.

Questionnaires for ADHD care for teens
• Patient Health Questionnaire—Adolescents (PHQ-A) open | close
This screen is available for adolescent self report of anxiety, depression and substance use and is thus also recommended for comprehensive ADHD care especially for screening for co-morbidity.
• Youth Pediatric Symptom Checklist open | close
This brief self administered screen has  attention, internalizing and externalizing subscales.

Collaborative Data Collection
• Clinician Invitations to Teachers or Teens open | close
Clinicians can email links into questionnaires from within CHADIS directly to teachers (or to parents who can send the link on to teachers), to ask them to provide input on a specific patient.  After the questionnaires are taken, the results appear immediately scored in CHADIS.  Now a link can be sent to teachers to complete multiple questionnaires, including the new tools noted above. 
• Teacher or Teen Visit types open | close
Specific visit types provide only the questionnaires specifically intended for the teacher or teen when they log in.

Monitoring
